SB 1383 Non-Compliance Complaint Form

  1. This form is for reporting noncompliance complaints regarding Commercial Edible Food Generators or Food Recovery Organizations or Services including local pantries.

    Your complaint will be kept confidential and can be submitted anonymously. Providing your contact information allows us to follow up with you if additional information is needed.

    All complaints will be reviewed and processed to the extent necessary. If we have information that a site is in compliance or the complaint is not a violation of the rules, a site visit will not be conducted.
